OREGON MEDICAL MARIJUANA PROGRAM
What it is
The Oregon Medical Marijuana Program (OMMP) is a State registry program within the Public Health Division of the Oregon Health Authority whose role is to administer the Oregon Medical Marijuana Act (OMMA) as approved by the voters in November 1998. While the OMMA does protect medical marijuana users who comply with its requirements from Oregon criminal prosecution for production, possession, or delivery of marijuana; the Oregon Department of Justice has advised the Oregon Health Authority’s Oregon Public Health Division that the OMMA neither protects marijuana plants from seizure nor individuals from prosecution if the federal government chooses to take action against patients, caregivers or growers under the federal Controlled Substances Act.
How to get a card

Attending Physician Statement. The OMMP card is reserved for patients with a qualifying medical condition; such conditions are documented using a form called the Attending Physician’s Statement, which must be signed by your physician. Keep in mind that the signing physician must be a Medical Doctor (MD) or Doctor of Osteopathy (DO), licensed to practice medicine in Oregon, and that you must have an established patient/physician relationship with them. Naturopaths, chiropractors, and nurse practitioners cannot sign your Attending Physician’s Statement.
OMMP Application Form. The other half of your OMMP application consists in the OMMP Application Form. This form also contains information that will help you determine the amount of your fee. Unfortunately, the application and/or growsite registration fees cannot be waived, and partial payments are not accepted.
Mail in your application. After you have filled in your OMMP Application Form and had your Attending Physician Statement signed, you are ready to mail your complete application. Make sure your complete application contains all of the following:
- Your filled-in OMMP Application Form;
- Your signed Attending Physician Statements;
- ID copies as requested;
- A check or money order covering the full amount of your fee.
As you send in your application, make sure you keep a copy of it along with any other information you send to the OMMP for your record. Mail in your complete application to:
